Application to register a patient permanently with your practice visit

please note if you are already registered with a practice in the area we are unable to take you on at this time, unless you have been asked to find a GP due to your address now being outwith their practice boundary
Registrations will take place Monday, Tuesday, Thursday & Friday 11am - 12.30pm
If you live in our practice area and would like to register with us please check our boundary with our receptionists. If you need to see a doctor urgently, please inform at time of registering and we will try to accommodate.
You will be asked to complete a registration form and new patient questionnaire. We also have an online access form should you wish to use this service for ordering prescription and booking doctors appointment. You will find these in the links below:
Please note if you return forms and our list is closed we cannot accept your registration. Please complete and return as soon as possible
What we need to see to register you
Proof of address – one of the following:
Council tax bill, lease agreement, mortgage statement, utility bill (gas, electricity) TV licence, electoral roll/polling card.
Proof of Identification - one of the following
We must see photographic ID - passport, ID card, driving licence/provisional
Overseas new registrations
We must have proof you are working/studying
Patients are registered with the practice rather than an individual GP, however for administrative reasons you will be issued with the name of one of the doctors. You can at any time express a preference for a particular doctor. Please let the receptionist know the name of the GP you wish to book an appointment with.
We recommend that where possible you see the same doctor for an on-going problem or illness. Not all the doctors in the practice provide all services and specific doctors may not be immediately available.
Changes to Personal Details
If you change your name, address or any other contact details please inform us immediately. You can download a form to complete here.
